The Transportation Safety Board of Canada (TSB) is issuing 14 recommendations as a result of its investigation into the May 2013 fatal crash of an Ornge Sikorsky S-76A helicopter in Moosonee, Ontario.

The investigations conducted by the Transportation Safety Board of Canada (TSB) are complex, and it is never just one factor that causes an accident. The December 2012 Perimeter Aviation crash in Sanikiluaq, Nunavut, was no exception.
On 22 December 2012, Perimeter Aviation Flight 993 crashed while attempting to land in Sanikiluaq, Nunavut. The aircraft had come in too high, too steep, and too fast, striking the ground 525 feet past the end of the runway after an unsuccessful attempt to reject the landing. The 2 crew and 6 adult passengers, secured by their seatbelts, suffered injuries ranging from minor to serious.

On 11 November 2007, the Bombardier Global 5000 (registration C-GXPR, serial number 9211), operated by Jetport Inc., departed Hamilton, Ontario, for Fox Harbour, Nova Scotia, with two crew members and eight passengers on board.
On August 2, 2005, the Air France Airbus A340-313 aircraft departed Paris, France, at 11:53 a.m. Coordinated Universal Time (UTC) as Air France Flight 358 on a scheduled flight to Toronto, Ontario, with 297 passengers and 12 crew members on board. Before departure, the flight crew members obtained their arrival weather forecast, which included the possibility of thunderstorms.
