The occurrence
On 6 February 2018, the fishing vessel Fisherman's Provider II, with four people on board, grounded on Frying Pan Shoal while transiting the Canso harbor, Nova Scotia. Three crewmembers were rescued by the Canadian Coast Guard auxiliary vessel Miss Lexi. The master perished after refusing to abandon the vessel.
No pollution was reported.

The occurrence
On 5 December 2017, the dredger FRPD 309 lost propulsion and collided with the barge EVCO 60 which was under tow by the tug Storm Wave along the Fraser River in British Columbia. The dredger and barge sustained damage.

The occurrence
On 15 November 2017, the roll-on roll-off cargo vessel Seaspan Swift, with 10 people on board, reported striking the No. 2 ramp while docking at the Tilbury terminal, in Delta, British Columbia. Minor injuries were reported as well as major damage to the vessel and ramp.

The occurrence
On 7 August 2018, the tug Ocean Monarch, with 3 people on board and towing the barge EVCO No. 15 made bottom contact in the Princess Royal Channel near Graham Reach, British Columbia. The tug's hull and starboard propeller nozzle were damaged. The tug continued on its voyage to Kitimat, British Columbia, using its port engine. There was no pollution reported.

The occurrence
On 06 March 2017 at approximately 1545 Pacific Standard Time, the fishing vessel Miss Cory, while seining for herring with 5 people on board, took on water, capsized, and sank in the Strait of Georgia, British Columbia. As the Miss Cory capsized, 4 people transferred to the fishing vessel Proud Venture, whose crew was helping to harvest the fish. One person who was in the engine room at the time of the capsizing was subsequently reported missing. Minor pollution was reported.

The occurrence
On 08 December 2017, the bulk carrier Federal Champlain reported that while docked at the Thunder Bay terminal in Thunder Bay, Ontario, the Third Officer fell overboard from a ladder into the water while reading the midship draught. He was rescued by a small vessel and sent to the nearest hospital for treatment. The TSB is investigating.
