The occurrence

On 13 April 2024, at 0206 Atlantic Daylight Time, the Keewatin Air LP Beechcraft King Air B200 aircraft (registration C-GBYN, serial number BB 1232), departed Goose Bay Airport (CYYR), Newfoundland and Labrador, on an instrument flight rules medical evacuation flight to Québec/Jean Lesage International Airport (CYQB), Quebec, with 2 pilots, 2 medical staff, and 1 patient on board. Shortly after the departure from Runway 26, the flight crew received a left-engine fire indication.

On 30 March 2024, at 1352 Eastern Daylight Time, the Boeing of Canada Limited de Havilland Division DHC-8-314 aircraft (registration C-GAIW, serial number 300) operated by Air Inuit Ltd. took off from La Grande Rivière Airport (CYGL), Quebec, to conduct a daytime cargo flight under instrument flight rules to Kangiqsujuaq (Wakeham Bay) Airport (CYKG), Quebec, with 2 flight crew members and 1 cargo agent on board.

The occurrence

On , an Aerospatiale AS 350 BA helicopter, operated by Aberdeen Helicopters Ltd., took off from Vernon Airport, British Columbia, on a visual flight rules training flight, with 2 crew members and 1 passenger on board.

After a short time in flight, the helicopter’s hydraulics were shut off to accommodate the training exercise. A few minutes later, the helicopter began to pitch nose-down.

n 28 June 2024, a Bell Helicopter Textron Inc. 212 helicopter operated by Great Slave Helicopters 2018 Ltd. was in the area of Fort Good Hope, Northwest Territories, supporting wildfire suppression efforts. Shortly after takeoff from Fort Good Hope Airport (CYGH), the helicopter experienced a main rotor blade separation from the head. Immediately thereafter, the remaining main rotor blade and the rotor head separated from the mast. The helicopter then collided with the terrain, resulting in a post-impact fire.
