Aviation Investigation A14Q0155
On 7 October 2014, an Air Canada Airbus 330, flight number ACA875 from Frankfurt, landed on runway 24R at Montreal/Pierre Elliott Trudeau Airport. Shortly after landing, the two outside wheels of the left landing gear bogy ran off the runway surface for approximately 550 feet before returning to the runway. The aircraft then proceeded to the skyway where passengers deplaned. Four runway edge lights were damaged. The aircraft was not damaged and there were no injuries.

Mr. Perrault has been a senior air investigator with the Dorval, Quebec, regional office of the Transportation Safety Board of Canada (TSB) since 1990 and has participated in several hundred investigations on air transportation accidents and incidents. In 1989, he joined the Canadian Aviation Safety Board as an investigator at the head office in the confidential report department. He received his commercial pilot licence — helicopter in 1974. From 1975 to 1989, Mr. Perrault worked mainly abroad.

Transportation Safety Board investigation process
There are 3 phases to a TSB investigation:
- Field phase: a team of investigators examines the occurrence site and wreckage, interviews witnesses and collects pertinent information.
- Examination and analysis phase: the TSB reviews pertinent records, tests components of the wreckage in the lab, determines the sequence of events and identifies safety deficiencies. When safety deficiencies are suspected or confirmed, the TSB advises the appropriate authority without waiting until publication of the final report.
- Report phase: a confidential draft report is approved by the Board and sent to persons and corporations who are directly concerned by the report. They then have the opportunity to dispute or correct information they believe to be incorrect. The Board considers all representations before approving the final report, which is subsequently released to the public.
